This template is designed to be copy-pasted into a doc or email so participants can fill it out and refer back to it easily. Whether you're a rep, manager, or facilitator—this is a great way to commit to change and stay accountable.
PERSONAL ACTION PLAN TEMPLATE
My Commitments for Practicing Carefrontation
1. One thing I will start doing:
(What’s one behavior, habit, or mindset shift you want to intentionally begin?)
e.g. “Start asking my team for feedback after every big meeting.”
2. One thing I will stop doing:
(What’s one unhelpful behavior that might be holding you or others back?)
e.g. “Stop avoiding tough conversations with my peers.”
3. One thing I will continue doing:
(What’s already working well that you want to keep doing consistently?)
e.g. “Continue recognizing team members for giving constructive feedback.”
4. Who I will give feedback to this week:
(Name one person and what the opportunity might be.)
e.g. “My AE teammate—during our pipeline review.”
5. How I will ask for feedback:
(Write out a phrase or question you’ll use to invite feedback.)
e.g. “I’m working on being more concise—did I hit the mark in that client meeting?”
6. How I will track progress:

Encourage your team to fill this out at the end of a Carefrontation session and revisit it a week later.
